An evidence binder is what turns a number into a documented number. Every figure in a reimbursement report should trace back to a source document the carrier can verify. A complete binder makes the claim defensible and signals that the underlying work was done carefully.
What to include
A complete receipt evidence binder generally contains:
- An indexed list of every receipt and bill
- Source document images or PDFs for each line
- Upload timestamps and, where used, OCR confidence notes
- Utility bills with billing periods visible
- Prior payment records and advances
- An exclusion log with a reason for each set-aside item
- PA review notes where methodology needs explanation
Index for the reviewer
The binder should be navigable: a carrier adjuster should be able to find the document behind any line in seconds. A clear index, consistent labeling, and ordering that matches the reconciliation schedule all reduce friction — and friction is what slows payment.
